Refusal of treatment and hospital transport.

For the first time I require bed to bed hospital transport to take me from hospital and back. Sometimes doctors who don't know me decide I should stay in hospital due to my general health and I have to sign myself out once my treatment has ended. If that happens do they still have to send me home laying down in an ambulance or do I lose that right?
