Hi Docjam55 ! Have you tried calling the MSAA helpline to see if they know if resources? Below is their contact info.
When I couldn’t handle stairs and yard work any more, I sold my house and moved to a duplex/condo. I loved my house and my yard but it became too much for me.

I moved to a very nice Condo building too. Even has underground heating parking which is awesome for Wisconsin winters. I used to have 10 acres in northern Wisconsin. Lyme and MS made that way to hard....
I live in NC, and I love my home and my yard and my garden. I chose to hire someone local to do the mowing, occasional pruning, etc. that I need help with. When I could no longer handle stairs, one of the Local Churches came and built a ramp for me, and made it possible for me to stay in my home, with the Financial assistance of both MSAA and MS FOCUS!

maybe contact some churches or reach out to a community service and they might be able to point you to the right direction
