Switched from Peg to Bes 3 weeks ago and had my first panels taken last week (although I am on quarterly reads at the moment, MPN doc wants to have reads after first 3 Bes doses to see if anything changes vs Peg)...
First results were great...all results (CBC, liver function, inflammation, glucose) were WNL with platelets at 320..not super surprised as imagine residual effect from Peg still likely also playing a role (last Peg was 5 weeks ago)....
Only outlier was that kidney function was down a little - but hoping it is because I have not been hydrating as much as usual (just have not been hitting the water like I usually do but now re-doubling efforts which is a little more difficult in the winter season)...
Currently on 70mg of Bes bi-weekly (was on 90 of Peg bi-weekly). Hoping pattern continues (and kidneys shoot back up)....now not likely to re-test AB until end of year (had contemplated doing it in June after about 3 years on Peg but my doc now wants to wait until either I am back on Peg or we have longer time on Bes)...

You may be right on lingering effects of IFN, my counts held for 3 months after quitting. 70 is a small dose of Bes, and if you can keep your counts in line with that, it is a good thing.
Yes, 70 is not a lot but is equivalent to what I was on with Peg - my MPN specialist wanted to start at 100mcg but was willing to let me start at the suggested conversion and see if things hold at the lower level...if need be we can increase (slowly) going forward...
That's good news , I've just switched to 125 mg Besremi last week and up to now I've been fine .
My consultant only wants me to take it once a month , she did say it might need to be injected every 3 weeks but wants to see what my blood results are in April.
Fingers crossed it keeps my blood levels ok because I've only just started responding well to the peg injection . The thought of injecting every month would be fantastic 😊
Hi wonderful community! I guess small achievements should be celebrated. Just got back my allele burden percentage result and having been at 94% when I was diagnosed in 2018, I am now at 90% after year and a half on Besremi and 6 months on Rux. Granted, I started at the lowest dose(50mcg) and am currently taking 100mcg every 2 weeks. I guess I was expecting to see a bigger drop, but aside from the small decrease, I am doing very well on both drugs and will take that as a good consolation " prize".
Hi Pogm, It sounds like you are on the right track. I take 500 of Besremi and have been on it for 2 years. It's just been the last 6 or 7 months where my allele burden dropped alot. 60 to 25. Good luck!
