Switching from Pegasys to Besremi

Just been to pick up my first couple of syringes of Besremi but Pharmacy couldn’t determine the equivalent dosage level. I showed them Hunter’s screenshot of Dr Mesa’s conversion table but they wanted something from the FDA. Hence asked me to come back tomorrow whilst they work on it.

Does anyone have any official links re dosage conversion?

There is no FDA guidance for switching from Pegasys to Besremi that I am aware of. That is why the recommendation from Dr. Mesa is so useful. I would suggest contacting PharmaEssentia Source pharmaessentiasource.com/ to see if they can provide documentation of the guidance.

Thanks for sharing some of your experiences w/ the "Switch-Over" to Besremi...

I am so glad that your NHS is covering, as my understanding is that the RRP for Besremi is equivalent to that of Ruxolitinib(?) As yet, our Australian PBS has not included funding for same...

Naturally, I am sure that many will be interested as to how your journey continues under the newer Med's etc.

My Pharmacy has concluded that Hunter’s screenshot from Dr Mesa is as good as it gets! I can’t see anything official from the FDA . As more of us switch from Pegasys to Besremi (now covered by the NHS) there must be some UK guidelines issued soon?

I’m on c 65 mcg weekly Pegasys and have been converted to 90 mcg Besremi every two weeks

Thanks Paul, hopefully we MPNers 'Down-Under' might also eventually have Besremi as an alternate to shore up the "Peg'" shortage come March 2025(?)

Unfortunately, in Australia, it will be yet another problem of an 'Economy of Scale', (just not enough Blood Cancer MPNers taking Peg' to warrant the expense), in my view...

Hope I am mistaken of course...

Hi Paul. Were you put on in any extra blood test monitoring. Just asking because I was on weekly testing when I was first put on peg, then two weekly. Might be useful data as well.
