I’ve read tha beliwvrange RBC can cau muscle pain and spasm.
Mine are always low and I hit lots of muscle pain and get soasms in my trapezes.

Suggest that you review your issues with a MPN Specialist. I believe that you are taking hydroxycarbamide previously. Sorting out what is the result of a MPN, other conditions, and the meds you are taking requires expert input.
You may need to ask specifically if the low RBC is causing the cramps.
I have low but not below range RBC and have found that magnesium supplements made a world of difference. Magnesium helps muscles relax. If you don't want to take the supplement without knowing if it would work or not there is a lotion called theraworx which contains magnesium. This lotion works very well for me before I started supplementing Mg. If the lotion relieves your cramp and pain then it could be that there is too little magnesium in your diet. From what I read, low Mg levels are fairly common and it is difficult to take too much so it is a fairly safe experiment.
I had painful cramps in the legs and toes and was told by my MPN specialist that it was due to low iron after years of phlebotomies. I was told to take Iron supplements which then just led to more phlebotomies.. although the painful cramps did reduce tremendously after several months. (The body absorbs iron very slowly). It became a vicious circle and thus the decision to start Besremi.
I had the same from low iron and found that vitamin E supplements helped. I was having bad restless regs at night and that completely stopped it. I also stopped phlebotomies and started Besremi.
