I titrated my Besremi to 180 from 150 over the last three doses (a bit of a story there for another time).
I assumed I'd need a phlebotomy as I hadn't had a CBC for 7 weeks and my Hematocrit was already 40 but it went down 2 points! Almost never happens without phlebotomy.
Everything dropped with the increased dose.
CMP all was normal with exception of Sodium was low again at 132, and for some reason my carbon dioxide was 21 which is below normal at this lab.
Updated: My HEM wants me to lower the dose back down a bit. WBC is low but the subtypes are okay but I have some room with my hematocrit.

My haematologist recommended monthly instead of bi-weekly of the last dose I used. I think that the Besremi/Phama-Essentia study had shown that once the blood counts stabilize, then a monthly dose will be or should be followed. My WBC, RBC, hemoglobin, and hematocrit all went lower than the normal range after 12 to 13 months of bi-weekly Besremi injections.
Those numbers look really good! It is great when treatment works.
I also tend to run a bit low in several WBCs, particularly LYMPH. NEUT tends to be borderline. Provided NEUT >1.0 and LYMPH > 0.5 then we just monitor and do not worry about the mild leukopenia.
Glad to hear that things are gong well with the blood cell values. In the context of your other medical adventures, it is nice to have something really positive to report.
So glad to hear this! wish it was my blood test. 🙂 I’m at 200mcg. 8 months of bimonthly injections. I’m one of the few ET patients who’s in Besremi. It’s doing it’s job slowly. New side effects come after raising the dose and they get less after each injection.
good news for you! My Besremi was dropped from 200 to 150mcg when my WBC’s went lower than the MPN liked. No phlebotomies since mid-Dec ‘22. Doing everything I want to.
Congratulations, I to continue to do well on Besremi my dose is 100mg. When I first started Besremi I also needed a couple of phlebotomies but have not need one in over 8 months.
