Normalization of CBC without medical treatment

Has anyone on this forum diagnosed with ET/PV with corresponding elevation in hemoglobin, hematocrit, RBC and platelet experience normalization (meaning CBC goes back to normal range without medical treatment)?

Hello! From time to time i have had normal counts, except my platelets that are always very close to normal limits (400-530)But i tend to get nervous cause i have read that this maybe sometimes a sign of progression to MF.. what did ypur Doc told you about?

With a primary PV profile with thrombocytosis it is most likely that a JAK2 mutation is present. I have never heard of a spontaneous remission of the JAk2 mutation. If it was actually Secondary PV or reactive thrombocytosis then resolution of the underlying disorder could resolve the elevation in CBC numbers. It is possible for someone with borderline numbers on platelets and RBCs to cycle in and out of the normal range, but this is not a remission of the MPN.

Any change in your MPN status would be something to review with a MPN Specialist. Hope you get answers soon.

Thank you for all the replies. I was dx with ET jak2+ in 2019. From that time my blood counts has been climbing gradually but never to where I needed phlebotomy. However in June 2021 my H&H was 16.1&50, plt 592 and RBC 5.47. It prompted me to see my MPN Dr which I did a week ago. There I had a repeat CBC and LDH. To my surprise H&H 14.5&44.5, PLTS 438. Spleen and liver size normal in june 2021. WBC stays normal. MPN Dr was not concern and said I should be happy with my numbers.

Hi, the blood counts can vary from month to month. It is the longer term trend that counts, I have been told many times. Mine acted like yours for my first 3 years after being told I had ET. They jumped up this spring and never went back down. I am now PV and taking HU. I wish I had your numbers without any treatment. I think your Doc is correct for now, just keep an eye on the trend over time. If you have not had your Epo level checked you might want to have that done. Mine was low when diagnosed with ET but the Dr did not pick it up or I might have been diagnosed with PV at the beginning. Although my BMB was read by two specialists and was diagnosed as ET. I had a H % H of 16.5 and 50.0 about 18 months before a formal diagnosis of PV but it went back down for about 18 months until jumping up to 17.2 and 55.3. I went 6 months without a blood test because of out of control covid where I live then had the jump up in counts. I hope yours stays down and you don't progress to PV. My specialist in mpn's says that 15% of ET will progress to PV. Who knows, it seems to me to be a wild unpredictable ride with any mpn. Best to you going forward.
