In discussing the pros/cons of moving from Hydroxy Urea to PEG Interferon, my MPN specialist citied one side effect of PEG as the possibility of activating latent autoimmune diseases that could be “brewing” in one’s system such as rheumatoid arthritis. This really surprised me as I thought the only side effects were fatigue and possible depression. My Dr did not see this as a reason to switch to PEG but I was wondering if anyone else had heard of this possible side effect?

It's certainly not unheard of - there are several papers out there saying that interferon can exacerbate pre-existing autoimmune disorders.
The MPN Voice leaflet on interferon alpha is here, and lists the most common side-effects. As with any drug, it's not a good idea to assume that you'll get every side-effect listed - the leaflet gives the frequency of each side-effect, which is useful information to have.
While not common, autoimmune adverse effects can occur with PEGylated Interferons. Both PEG and HU can have significant adverse effects. Both can also have benefits. Here is some basic information on both.
You have to weigh the risks/benefits of each of these medications. This has to be considered in light of your treatment goals, priorities, and risk tolerance. It is your preference about which treatment option you prefer based on the benefits you are seeking and the risks you are willing to accept. "Hydroxyurea is a highly toxic medication with a low therapeutic index." Pegasys is also a medication with significant potential adverse effects. Both medication can help you. Both medications can hurt you.
Which is best is a highly individual choice. In addition to preferences, we each react differently. I am HU-intolerant but am responding quite well to PEG. Other people find precisely the opposite and seem to do well on HU. If you are having problems tolerating HU, that is a valid reason to switch to PEG. If you prefer the risk/benefit profile of PEG, that is a valid reason to switch. If you are tolerating HU and benefiting from it, then there is no need to switch unless you prefer PEG.
Hi,Yes I think this is true. My thyroid gland has stopped producing hormones so my metabolism is too low and my cholesterol is out of balance as well. Apparently if you have this predisposition the interferon can cause it. I’m on Pegasys and have started 9 months ago.
