Combined antiplatelet and anticoagulant therapy for ET clot prevention.

Hi All,

Has anyone had experience with combination therapy for prevention of clots with ET or PV? I was switched from Clopidogrel to Apixaban after a suspected mesenteric clot and ischaemic bowel bleed. I had been stable for a long time and platelets still just over normal levels but something changed and problems started to arise.

The switch to apixaban seemed ok at first once I got used to it but then I started having periods of platelet showers ( still not remarkably high numbers) and symptoms of TIAs. I am now trying apixaban and clopidogrel combined. This will increase my bleeding risk ( esp with gastric varicies from a previous portal vein clot when I was a teenager) but I can’t wait for a major stroke event so here we are. My haem said they are now trying apixaban and drugs like it with aspirin in a COMPASS trial with success but wondered if anyone with MPN has experience with it? I am suffering some side effects today but it is day 1 of combined so they may settle with time. Any experience or advice on what to watch for welcome. My haem is good and I have his mobile for any concerns which is very good of him.

Hope everyone is maintaining an MPN balance at the moment.

Hi. I’ve been on rivaroxaban ( its a once a day drug) as an oral anticoagulant for 5 yrs or so: I had a pulmonary embolism before I was diagnosed with MF.

Very occasional nose bleeds, but no real probs.

Thanks for the info. Do you also take an anti platelet drug or aspirin as well as rivaroxaban?

No : the rivaroxaban seems to do the trick and keep my platelets OK .

I have also been taking rivaroxaban daily for 3 years following a pulmonary embolism. Have had no problems with it. Stopped taking aspirin when started the rivaroxaban. Kind regards
