Just woke up 10 minutes ago on day 7, I started using underware for the last 2 days and I dont know if Im going crazy but it hurts a little more on specific spots I popped stitches. Im starting to get worried its really infected, should I be concerned enough to call my doctor? Washing 3 times a day and putting on antibiotic cream each time.

The wound is still quite new. I'd cover the wound area on some gauze or dressing to decrease the friction of undies. The dressing supports the wound and keep the temperature more optimal for faster wound healing, too.
It looks fine and you are using an antibiotic cream to help stop infection. If you are in any doubt, however, see your doctor.
