Diagnosis Please

Hello all,

I am currently 21 years old, and I have just been experiencing different changes that I will mention below in hopes of getting an appropriate diagnosis.

I started getting really bad knee pain, joint pain, and swelling of the fingers especially at the PIPJ with redness evident at the PIPJ. I have been to a rheumatologist, and they have screened me for RA, and my RF came back negative. I also was tested for ANA, which came back positive with a homogeneous pattern (1:160). It was brought to my attention that I also have 5 swan neck deformities in my hand. When I did my recent blood work, I noticed that my creatinine levels decreased from my last blood work 6 months ago (.8 to .69) and that my Anti-DSDNA that was negative 6 months ago is now in the indeterminate level (6). The weird thing is that now my ANA is negative. I simply cannot do the things I enjoy like working out because my legs give out very quickly especially when going on the Stairmaster. I do wake up with my hands stiff every morning and my joint pain seems like it comes and goes.

I do have a family history of Scleroderma, RA, and Raynaud's.

Any thoughts? My PCP thinks that it might be Myositis, Scleroderma, Raynauds, or RA. Any thoughts please share.

I have attached a picture of my hand (keep in mind I am 21 and it looks like I have older hands)
