why arent you receiving some other treatment like ivig or nplate to raise your platelets instead of transfusions?? are you being treated by a hematologist that knows enough about itp?
while a count of 5 is considered to be dangerous for everyone, it is the most dangerous for people who have wet purpura and bleed (you has blood in your stool) so please consider doing something about your platelets.. a count of 5 in people without symptoms can be tolerated (i personally rarely bruise with a count of 5).

It depends. I had a count of 1 at some point and aside from a few bruises everything else was fine. However in your case, you should try something like NPlate or IVIG / steroids to increase your platelets, since you do have more severe symptoms. Transfusions tend to be much less effective (increased the count by 2 and only for one day)
I always got the petachiae in my feet first and then my neck when my platelet count was falling below 10. I had 7 lots of different treatments until they found the one that worked for me. What else have you tried? Blood in your stools is a concern.
