Is this an academic survey, or sponsored by a drug company? Where is the company based? Your email address doesn't make sense, surely it should be research in the suffix?

Wouldn't it be better to go via the ITP Support Association rather than a board like this, as otherwise you might be seen as just phishing for information on people? I don't share personal data with randoms on the internet.
