How long does it take to decide if it’s ICU delirium or brain damage, after coming off the ventilator?

My mum spent 17 days ventilated for covid and pneumonia and potentially a secondary bacterial infection. She was on CPAP for 2 weeks before and managed at home for 2 weeks before that. She suffered some kidney damage (no dialysis) had brain swelling (PRES) and some seizures.

They extubated her on Thursday and today she has her eyes open and is agitated but not following commands or responsive, nothing verbal and very confused. They had to give her a sedative so she didn’t injure herself when thrashing around on the bed.

So for 3 months post coma, I thought I was dead and this was just a dream. Even today, 4 years post coma, I tried to explain to my parents that I sometimes don't know what is real anymore, and what was coma nightmares. They assured me this is real, and I know deep down it is, but the feeling of having to ground oneself all the time, is exhausting and extremely distressing for the patient. She is likely still full of medication and in a waking dream / nightmare. It will take time, be patient x

Have the staff done CT or MRI scans?

No, I think CT scan is imminent but they wanted to hold off as long as possible because they would have to sedate her for it; which could be counter productive to the delirium.

They have said an MRI isn’t necessary at this stage.

Ok - why sedate to do CT scan I wonder?

They need the patient to stay still and if she’s agitated that would be difficult.

Yes that’s right they are having to give her mild sedatives to stay calm right now but would have to give her more to stay still and calm for CT.

It took my mom almost 3 weeks to show signs of conciousness. She's great and fully understands now.

They didn't make her any neurological tests. Just gave her time. She went through delirium but is great now.

Just wanted to update here that yesterday my mum began talking although nothing makes sense she shows signs she is understanding some things and has pronounced a few clear words. She goes up and down in terms of distress and agitation but hope we are moving on in stages of recovery. She’s still in ICU but remains stable.
