Labour and Hughes

I have Hughes and am treated with clexane x2 daily and aspirin. wondering if anybody has had this through pregnancy and whether or not it effected delivery of the baby? Was you induced early? would be grateful for advice as 35 weeks preg now.

My daughter was put on Clexane throughout her pregnancies, and was just told to stop when she went into labour, her labour was only six hours with her first but there was no problems with excess bleeding.

I took Fragmin and aspirin through both my pregnancies, and each time was induced slightly early (38/39 weeks) to make management of the deliveries easier. I was swapped from my daily Fragmin to a continuous infusion (not sure of technical terms, there was a pump thing attached to a cannula in my hand). I had no problems APS-wise or bleeding-wise with either delivery, and used TENS machine and gas and air with my daughter, and those plus pethedine (sp?) with my son. I couldn't fault my care with either pregnancy and delivery.

Hi, I'm pregnant on Fragmin and I had an appointment on Wednesday about labour. Wont be induced early but would only go 1 week overdue. The only problem with labour is if injection had been taken within 12 hours (I take 1 5000 injection in evening) then an epidural is out of the question. If I needed an emergency c section it would have to be under a general. Outside of 12 hours Id just be like anyone else so hoping that I go into labour at an appropriate time just in case I want an epidural. Within 12 hrs epidural is dangerous because there is no space around the spinal cord for blood so any build up of pressure is very dangerous. I have been offered remifentanil as an alternative to an epidural by the anesthetist at the hospital I will deliver at. This can only be offered if I have seen anesthetist so I would ask now what the policy is at your hospital. Good Luck!

stop fragmin 24 hours prior to delivery Bolus of heprin given slowly if a major concern in my case bilateral pe's 1 month before delivery even thou prophlactic fragmin throughout pregnancy -

I was induced so i would definately be at the hospital in time for delivery under high risk consultant care

I have had 5 babies and can safely say no problems with labour at all if there are no complication like my PE'S DON NOT HAVE EPIDURAL FOR PAIN RELIEF !!!!!!!!!!!!!!!!

Contraindications with fragmin !!

In my last labout I have to have emergency C SECTION and they were trying to puch me to have epidural (they obviously had not read my notes) If god forbid you need c-section make them knock you out !

This also makes it easier for them if you bleed during surgery

Thank you for telling me your experience.

I'm hoping to be induced so that if I so choose on the day I could get the epidural.

Some of the drs I see say there is a risk of bleeding in the labour and some don't.

I am so happy to see you all had healthy babies and that your labours were ok

I was told that heparin/clexane/fragmin are all 'quick' forms of anti coagulation which is why they are used in pregnancy, lasting only 24 hours which is why its a daily injection, its not long lasting so therefore shouldnt interfere with beeding levels, and cause any problems with delivery. Advisable to stop the day before delivery - but not life or death if you dont. I had 4 pregnancies on it and no excess bleeding. There is counter medicine they can give anyway if there was a problem. Aspirin builds up in your system like warfarin does and stays in your system for a long time so stopping the injection for one day isnt a problem as your still covered with the low dose apsirin which will stop any effects of the APS for that short period of time. Ensure you take the heparin/clexane or whatever else your on for 6 wks after delivery as your still at risk of the APS and clotting problems for that period of time after - this is my experience after consultant advice. I had a c section last time which is more risky than induction labour and i was absolutely fine x

Sorry,forgot to say - I was induced early as growth restriction of the baby - this will have been monitored on growth scans so dont worry yourself about it. Unsure whether this was due to APS or the heparin - but very rarely they let you go full term for better management. I asked way in advance for a date of induction and it was always no later than 38wks - so ask them and pin them down & good luck xxxxxxxx

We keep saying clexane is a miracle grow as this baby is on track to be 9 and a half pounds :-)

I inject every 12 hours so this is why I want the induction so that I can stop just before.

Ha ha. My baby is "above average" on growth scans so far, was thinking the same thing about it being a miracle grow! Wish I was being induced a bit early but been told unless growth slows down significantly that I wont be. Hope you will let us know of baby's safe arrival x

I was told that an epidural was a definate no-no. After having seen the size of the needle that is used for it, I was glad that it was a definate no-no!
