Welcome to the group.
Have you been told by your doctor about carb counting? If you do counting carbs, it can help with the weight loss issues and blood sugar control. Are you type 1, 2 or LADA (1.5)? Are you on insulin along with other medications or just medications on their own?
I'd head for a whole-food plant based diet. Legumes will bring blood pressure down - without meat and without dairy your blood pressure problems will be a thing of the past.
However be very careful as you start since, you must drop meds simultaneously with advice from doctor because taking blood pressure tabs at same time could cause too low blood pressure. Taking blood pressure tabs with normal blood pressure can be life-threatening. So make sure your doctor is monitoring your blood pressure as it drops to normal and gets the right amount of blood-pressure tablets for you.
btw assuming your diabetes is type-2 the whole-food plant based diet will sort that as well.

About six doctors in the USA advocating this method, see Ornish, McGregor, McDonald for a start plus oil free vegan for recipes. Diet seems a bit daunting at first but I have been on it for 18 months with just a little oily fish. Do not even fancy meat or chicken now. Let me know how you get on.
