I suffered a one-off seizure in January this year. DVLA were informed and sent me a Form FEP1, Consent Form and Consultant's Reply Form. I was advised to complete the med and consent form and then pass them to the Consulantto complete the Reply form to accompany my relicensing application (after 6 months). I passed the forms to the Consulant via an Epilepsy nurse whilst at a scheduled appointment. The form was returned UNSIGNED with a hand written note to DVLA Medical Advisor saying that the Consultant "would be happy to sign any forms if these are sent direct as is the usual practice". Unbelievable! I have been in email contact with the Consultants secretary and she claims they've never seen the forms before. How uncooperative and inflexible is this? This woman (I won't name her) is responsible for my care. God help me!

When I had to surrender my licence because it was thought I might have had subclinical fits when in a coma, although there was no visual fitting observed, and at that time the hospital I was in had no facility to do an eeg, and I was too seriously ill to be taken to a London hospital for it to be done. The DVLA sent forms that I had to complete myself and they did send direct to the consultants involved in my care, forms for them to complete. I can understand that because if the Consultant fill a form that you give them and then it is given back to you to submit, there is a risk that the person could decide they didn't like or agree with the info and could falsify and submit info as if from Consultant. Also if if sent direct by DVLA then no cost is occured whereas if we ask for forms to be completed sometime there is. If you have had a fit usually you have to be fit free for a year before DVLA will allow you to drive.
