I think it does depend on the type ie trauma/tumor/aneurysm and thus the risks, plus the degree of disability one has now, thus risks etc.
I went on a short holiday 11 months on, was more expensive but not massivly so but doesn't affect now 2 years on.
I agree with Roger. It depends on type of bi and its effects.
Be honest with the insurace company. I ( ok my wife) usually has to spend ages on the phone to them explaining bi to them and how it effects me. Usually it in the end there is no extra cost.

I had a traumatic BI in January 2015 in a car accident in Mexico. That September I flew to France by myself to attend a friend's wedding. I had to pay more but for my insurance as up to a year after my accident I was considered high-risk. I'm now past the year cut off so my travel insurance has reduced significantly. I called the Headway helpline for a recommendation of a travel insurance company for my trip to France and used their recommendation. But now I just search for insurance on moneysupermarket and am completely truthful about what happened. I managed to get annual travel insurance with Coverwise for a very reasonable amount. The insurance is underwritten by Axa. I recommend finding insurance underwritten by them.
Please DO NOT use Insure and Go. I previously used their annual policy and was insured by them when I was travelling in Mexico. They are truly terrible and will try to get out of everything. They caused my family a lot of stress. They still send me threatening letters trying to get me to pay for being airlifted back - which they insisted happen because the hospital in Mexico was costing them an absolute fortune. (sorry vent over!)
