Knowing it's time?

How do you decide that living with your family is unfair to me and them, and it's time to think about a home. How do you find one?

By "home" you mean a nursing home? I can not imagine what can be so "unfair" for my family...

Maybe, start with talking with them. Have a neutral friend there so it doesn't decend into a reflex "No, it's OK". Good point is no assuptions honesty, cause if a nursing home is the next steps you need them as guilt-free allies beside you in deciding which one best meets your needs. You and them probably know them best. Just don't start this next step by isolating yourself.

A very difficult decision...based on your age, their situation, whether someone has to stop work to look after you full time, your levels of other support.

May I suggest that you reach out on Facebook to find others with FND (Functional Neurological Disorder). The group has over 430 people, a couple who are almost completely paralysed from the neck down, many in a wheelchair, completely dependent on others and many who have full time carers. That Facebook group also has carers who are more than willing to offer support to sufferers and their carers.
