My S PSA report came today as 8.17 U/L.

Kindly advise me what to do now.

Should I go to Shaukat Khanum Memorial Cancer Hospital ?

Dear Ashra. I know nothing about your health services, so cannot advise you. I also don't understand you test result. PSA is measured in ng/ml, that is nanograms per millilitre. I've no idea what U/L means. However, if it were 8.17 ng/ml, it would suggest you may have cancer. You need further investigations, e.g. a biopsy. I hope this helps.

U/L is microgram per litre, which is the same as nanogram per millilitre, so the numbers are the same.

Mr Ashraf,

If you are otherwise healthy you should get it investigated. Only about a third of PSA scores in between 5 and 9 are prostate cancer, but at that stage the cancer is curable - if you leave it for a couple of years it may become incurable.

I looked at the web site for Shaukat Khanum Memorial Cancer Hospital and it sounds like it has all the equipment needed to investigate well and treat if necessary.

Please be aware that small amounts of low risk of spread prostate cancer (Gleason score 6 in 3 or fewer biopsy cores) in a biopsy are common and do not need treatment because usually they are not a threat to health. Gleason score 7 or higher is a risk to your health.

Shaukat Khanum Cancer Hospital is successful & capable to handle later conditions.

When you get a biopsy result, they will give you a Gleason score which is an indication of how cancerous the cells are. I believe 8 or more is a high risk, but 6 is an intermediate risk. You should also be told how many cores (out of a total of 12) are affected. I had 3 out of 12. But they could feel a lump also on rectal examination so this was a "T2". My PSA was 6.2. You should also have a bone scan and MRI. These will see if the cells have spread beyond the gland. Mine hadn't. They advised me to have surgery or radiotherapy. I chose surgery. Contrary to what others say, I'd take the advice of the doctors. It seems to me that with a PSA of 8 or more, a Gleason of 5 or more, it IS a risk to your health. The danger is, if it spreads beyond the gland, it becomes incurable. Ask the doctors many questions and find out as much as you can and take their advice, not mine or anyone else on this site.

So kind of your sincere advise.

They haven't done latest rectel examination.

On coming Tuesday I will go in OPD & request for proper examination & treatment by medicines if they agree.

Since this is an Government Hospital's OPD they can't give their full attention.

Good Luck. Your prostate may not necessarily have shrunk on rectal examination, but the lower PSA is still a good sign. Have they ever tested your urine? If you have infection, there will be white blood cells in your urine. You should have three samples tested. 1 is the first few mLs you pass. 2 is separately the rest. These both BEFORE rectal exam. 3 is AFTER rectal exam. If these samples are positive for white blood cells. If 1 is +ve and 2 NOT +ve then the infection is in your urethra (unlikely in your case.) If 1 and 2 are both +ve then infection could be in prostate or bladder. If 3 is +ve then the infection is in your prostate.

Protein, Leukocytes and red blood cells are not normally found in urine. Leukocytes definitely indicates inflammation. The other two aren't quite as definite, but can be present with inflammation. The 3 urine tests could help identify exactly where the inflammation is urethra bladder or prostate, but the DR may not think it necessary.

Sorry, I have no photos of saw palmetto. You'd need 1 to 2 grams a day of the herb itself. Best to get standardised tablets. It does have other effects too e.g. on hair.

Don't worry too much about the urine tests, the point is the medicines you're taking seem to be working. A repeat simple urine test would confirm this. Leukocytes, blood and protein should also reduce or disappear.

Yes! Go to the best that you can..Find out your total status asap, and start, if you havent already to take care of your body and heal.Its possible to put that PSA down and also hold off even apc for some time .If we can do that? There are new rapidly drugs and hopefully better treatments are in the pipeline.But for us now there are few treatments usually used.The main 3 ; R. T. ,Chemo, and ADT all ravish your very existence..With time we can adjust to a new lifestyle ...many men on this site have lived long fruitful life's with apc.Im just 2 yrs in on my voyage..It's a lifetime affair with apc. Sooner the better that one accepts the diagnosis and takes aggressive measures, but of course that depends on your particulars and severity.Hopefully you are not advanced, there is much success with early treatment.Come back with your Gleason score and if there are mets..Then the guys here will flood you with so much knowledge that should give you an informed shot at a successful plan. Don't let fear destroy you..There are a lot of things you can do to help youself.

My 2nd PSA test report is due on June 16,2017.

If you can laugh and cry you can survive , Our perseption and attitudes can save us or bring us down. . I battle daily to be positive But I'm lucky because I also have great moments of joy each day. Eliminating stress is key. That's Easy to say .........Wishing you good health. Hang in there!

I have just come back now after attending Jinnah Hospital OPD & met my Doctor & requested to continue Tables Ciprofloxacin 500 mg but Dr. didn't agree because in his opinion PSA may come down further which will miss guide us.

He urged to Continue for 3 Months 1 Capsule Tamsulosin 0.4/0.5 mg daily only.

Called me now in OPD after 3 Months.

What do you suggest now.
