hello\ Emma this ius not the same as the ESA questionaire I received.is that for making a new cliam?i am on IB and transferring to ESA .myhtoughts are why are thye making people fgo through all this stresswhen they have already been declared unfit for work?

Just to add you are able to download the form, type your answers then print, which is easier for people that may have trouble with writing due to wrist pain etc. You must remember however that the last section, the declaration needs to be signed and dated by hand before sending. Hope this helps
