Lost and Anxious - Low AMH, Mild adenomyosis, borderline male infertility

Hello everyone, I hope you’re all as well as can be.

Apologies for the long message! I am really lost and my anxiety of making the wrong decisions is stopping us from moving forward!

I’m 34, my husband is 39, we conceived naturally in September 2019 and despite pregnancy complications, we were blessed with our daughter in June 2020.

We have been trying to conceive for 21 months. We tried at home insemination kits for 5 months.

I have had scans and tests which have identified mild adenomyosis (the consultant believes i would have had this before and during my first pregnancy so doesn’t think it’s the cause of our infertility)

The scan showed just 3 mature follicles on the left and 7 on the right with a few more on both sides above 9mm.

Bloods revealed low AMH levels of 3.9.

Otherwise everything else looked normal, I have a regular cycle and appear to be releasing an egg each month.

Husband’s sperm count 180 million and very high mobility but morphology was just below 4%.

We have been advised to opt for a three cycle plan with ICSI.

The consultant said that if I wait more than a few months my fertility will have depleted significantly, so I want to try and choose the right clinic, ask the right questions to increase our chances of getting the treatment that suits our circumstances (many reviews of clinics suggest people feel their treatment wasn’t tailored to them at all).

I would be so grateful to hear from anyone with similar fertility issues and any tips on what to ask, what to be mindful of when choosing a clinic, or if there is an approach you would recommend in terms of the medication, back to back cycles - anything really!

Hi lovely, I'm sorry to hear you've been struggling. 💛 I would strongly suggest having your partner do further investigations like a sperm DNA fragmentation test given his low morphology result. Sperm issues are one of the most common causes of secondary infertility. This is because a woman's eggs have the ability to repair damaged sperm but as we get older our eggs have less ability to do this, so problems with sperm that may have always been present but were 'fixed' by the egg previously, suddenly become more of a hurdle when you go to try again a few years later.

If his results come back with high DNA frag it will change how you approach IVF (you'll want to try to figure out the cause of the frag first of all), so it should be your first step before starting any IVF cycles.

not any advice but thought I would say hi as our results seem to be very similar. My husband morphology was 1% and I had low amh. I only had 4 eggs retrieved, only 3 matured and only one fertilised. We had ICSI. Hope all goes well with your journey. X
