Don't know what to do anymore 😭 soiling

My daughter is 4 and half, started toilet training about 2 years ago. The majority of the time has been great with wees, the odd blip here or there but other than the occasional too engrossed in play she has been great. Has never mastered poos on the toilet consistently though. Been treating the constipation for over a year with limited success. Most recently tried two lots of disimpaction. Second time we kept going at the highest dose for her age which was 8 Movicol and 5ml of senna. We got to water with bits but never like the video that I have seen on Facebook from Dr Kate McGarry. But she was having lots of liquid poos that were going through everything. Through disimpaction she would easily be having 10 bowel movements a day. Some on the toilet and some in pants. Some she would tell you she needed a poo, others would just happen when she said she was going for a wee. She would sometimes realise that she needed a poo whilst she was there so it's not like there is no feeling there. We are trying a maintenance dose of 4 as suggested by the poo nurses and she has been on this since Friday. But today alone with have had 5 accidents and some poos on the toilet at nursery. They are mainly just bits. Is it the bowel settling down or a sign it hasn't worked? The videos make it look like a simple process and that it just works but the reality couldn't be further from the truth.

Just to also say we have paid to see a private paediatric doctor who didn't help a great deal. We have tried the usual timed toilet sits, sticker charts etc. GPs have been useless, never seen her in person and I have been the one to suggest disimpaction etc to them. She starts school in September, the whole family are impacted by it, I'm really worried about her and having a serious impact on my mental health as I feel alone fighting a battle with no help or answers. Should accidents stop after disimpaction or does it take a while before this happens?
