Withholding poo

My Grandson age 3 (who is on the spectrum) has been withholding his poo for approx 2 years. When he does eventually manage to go (roughly every 3/4 days)his poo is huge and it's extremely difficult for him to pass! He's on 2 sachets of Movicol a day, but I don't think it's doing any good! We've also tried lactulose, inulin and Docusol, as he has an intolerance to strange flavours, he refused the latter two! Doc refuses to prescribe a stimulant because of his age! Because of his condition, he isn't good with communicating, so toilet training at the moment is a complete no no! Any help would be greatly appreciated!

With regards to the laxatives, its important to make sure that you mix them with exactly the correct amount of water, before adding to any other drinks.
