Medical evidence for endometriosis and adenomyosis

Hi everyone, so I’m in my first year of university and I recently told my tutor that I have endometriosis and adenomyosis. She was really understanding and supportive and recommended I got in touch with student disability and dyslexia support (they also handle chronic and long-term illness) so accommodations can be put in place. I’ve just received an email from them to arrange a meeting but in the email they’ve asked if I had any medical evidence of my diagnosis. I was verbally told I have endometriosis and adenomyosis so I don’t have any proof with me. How do I get medical evidence of my diagnosis? It was my gp who referred me for an ultrasound and it was him who told me I have endo but he then referred me to a gynaecologist who had a special interest in endo and they again told me I have endo but also adenomyosis. I’m not sure how to go about this, so I’d be grateful for any help or advice. Thank you x

You can apply for copies of all your records under what is called a subject access request, for which they have 28 days, but you should be able to access your GP records online. I should get in touch with your GP.

Your doctor should be able to provide a letter with diagnosis and that referral to specials to determined level of diagnosis. Poss future treatments.
