No it cannot come back provided all the existing endo was removed at the same time as you had a hysterectomy.

But if your surgeon did not remove all existing endo at the same time as giving you a hysterectomy then yes it will remain there and active and responding to changes in hormones from the ovaries, or HRT, and the foods that you eat and medicines you take that might promote oestrogen production.

Oestrogen is not only from ovaries - but also adrenal glands and tummy fat which is also a warehouse for excess oestrogen. So even when ovaries are removed along with the uterus and patients are put on HRT, that is enough of a supply of oestrogen to activate endo.

Unfortunately we all need oestrogen for bone growth and heart health and cannot be well if if we cut all production of it, so the next best option is always to have existing endo removed so that it is not there to react to the oestrogen.

Provided there is a ready supply of oestrogen and provided you have existing endo then it will be able to behave as it did before, bleeding and irritating nearby tissue and causing pains.

Another factor that might be an issue after a surgery including a hysterectomy is scarring and adhesions which can form on surgical sites inside th body and keep growing and cause all sorts of problems over time. Even when all endo is removed the body can still produce adhesions and will still form scars - and you cannot do anything to prevent that occuring but perhaps occasionally have an op to cut the nuisance ones back and free up organs when they get stuck together.
