Pelvic congestion is a separate condition to endometriosis.

Both are made worse with oestrogen - but in a different way.

It is a problem with the vascular system - the veins running through the pelvis become varicose or twisted and swollen as the result of poor circulation, which can be caused by oestrogen dilating or stretching the veins in odd places. There can be other causes too like pregnancy.

Same can happen in legs and in bowel (causing piles).

Not everyone feels the varicose veins as sore. Some do and some don't.

Just as with piles, not everyone has painful piles, not everyone with varicose veins in the legs will be in pain and not ever woman with varicose veins in the pelvis will be aware of it.

If they are causing pain all the time then they can be attacked by a surgeon.
