Swallowing on both feet and pain in muscle of fore legs, thigh and waist.Solicit medical advice as to how to get relief?

I am 69 years old. Suffering from DM2 since 1998 and started medicine. Taking insulin for the last 4 years. Presently taking NOVO MIX 30 penfill., 22 units before dinner and 18 units before break fast. Sugar as measured by glucometer: fasting 135 yesterday.

I am observing swallow in the feet of both legs associated with pain in muscle of both leg and waist.
