I am getting my teeth cleaned (scaled) next week, it's been 6 + years since I had it done. (prior to CLL dx) . It's a new dentist , I explained about having CLL. The prescription reads 500 MG , Amoxicillin, take 4 , One hour before cleaning. 2000 Mg in my gut sounds too high to me. Due to not having major issues I cancelled the oncologist this past March. I did some research but not getting the answer I feel I need. Just The thought of sitting there with 2k mg of amoxicillin in my gut at once is already making me feel Queasy Is there a recommended dose before and after dental procedures ? I have been on W&W since Dx 1/23 . Last CBC 8/16/24
WBC = 13.8
ALC = 8156
Monocytes 5/23 = 173 6/23 = 636, 9/23 =515,
1/17/24 = 594 5/17/24 = 664 , 8/16/24 1007 π
Everything else is WNL
Appreciate any thoughts or suggestions . ππ

Scaling is more invasive & potentially exposes your blood to more mouth bacteria than a simple cleaning.
You haven't posted your platelets, so I can't comment if that number is a consideration. Or your immune globulin level. Or any other symptoms or labs you may have.
Ask them why they think you should have it! It may be as simple as "doc noticed leukemia in your diagnosis & is using immune compromised patient protocol".
A Continuing Medical Education credit article from 2023, states there is a lack of evidence for patients with immunodeficiencies (#35)
Due to a heart murmur I have always been prescribed a prophylactic antibiotic . I was the one who requested the antibiotic for this reason and also I have some heavy tartar. I expect some heavy bleeding.
Since dx my platelets have been bouncing around and heading south , 2/23 = 188 , 5/23 = 202, 6/23 = 176, 9/23 = 180 ,, 1/2024 = 174 , 5/ 2024 = 191 , 8/2002 = 145
2/2023
Gamma Globulin = 0.7 , Immun. A = 58 , Immun. M = 46 , Immun G = 801
I needed antibiotics for a bug bite and an eye infection this past summer. Laryngitis for 2+ months, ( no results with scope) MRI is scheduled next week to check for a slip disc , been going to the chiropractor to help with lower leg weakness and fatigue . Thank God no Temps. Like every one just rolling with the punches , And being thankful b/c I know it could be a lot worse.
Good article , Thank you ππ
Thank you for your feed back.
Hope this goes through , having trouble with HunL site from sunny Ca ,
I have my teeth cleaned roughly every 6 months. I'm on W and W with WBC40 Lymphs 36 Platelets around 200 and have never had antibiotics for a scale and polish. I have had CLL for 16 years. never had treatment
I consulted my hematologist about having crowns done on my teeth and if I should have antibiotics. He said I should only have antibiotics for a root canal.
I've had 16 teeth cleanings and one filling without incident since I've been diagnosed (mostly on treatment). My numbers were probably way higher half the time.
I just had some major dental work done and my dentist spoke with my oncologist before doing anything. I went off Ibrutinib 5 days prior to my work and yes took a loading dose of 4 Amoxicillin pills before anything was done. So happy with my work and my doctors working as a team. My dentist said he learned a lot after speaking with my oncologist and it will help him with other patients. Good luck. Sally
I have antibiotics everytime I ho yo the dentists, but I had my aortic heart valve replaced and my surgeon said I needed them before any dental treatment to protect my heart. I informed my haematologist when starting treatment for CLL and she just said that was good. That dose doesn't hurt you at all.
CLL - compromises immune system therefore always advisable to take antibiotics before dental treatment, I insist upon it when hubby as dental work done however I cannot comment on dosage so ask your pharmacist.
There isn't consensus on antibiotic prophylaxis, among CLL specialists, but my haematologist recommended this for me because I had chronic neutropenia.
I have this done regularly because for some reason I make a lot of plaque! My dentist does not give me antibiotics and says that she was advised I would only need them for a root canal should that be necessary.
I have a congenital PFO (hole in my heart). Have always taken 4 giant amoxicillins before dentist for over 50 years. No problems at all. Ever. Important to keep those teeth up to date.
