I would consult with the oncology pharmacist about this. The ones that I deal with are well versed in the supplements I take and some were not recommended. I am on Acalabrutinib.

According to MSKC's integrative-medicine/herbs database, mskcc.org/cancer-care/integ...

Saw Palmetto interactions are possible with:-

Anticoagulants, antiplatelets: Saw palmetto may have additive anticoagulant effects, according to case reports (11) (21) (49).

Nonsteroidal anti-inflammatory drugs (NSAIDs): Saw palmetto may increase the risk of side effects with these drugs, according to a case report (21).

UGT (Uridine 5’-diphospho-glucuronosyltransferase) substrates: Saw palmetto inhibits UGT enzymes in vitro and can increase the side effects of drugs metabolized by them (25). Clinical relevance is not known.

CYP 450 substrates: Saw palmetto inhibits cytochrome P450 3A4, 2D6, and 2C9 and may interfere with the actions of drugs metabolized by these enzymes (29). Clinical relevance is not known.

The latter point about CYP550 substrates means that as tovah replied, you should check with an oncology pharmacist about possible interactions with ibrutinib and venetoclax, as it could worsen side effects without dose reductions.

I was taking it for an enlarged prostate. I had to stop taking it before starting V&O. Fortunately found something better at half the cost😀.

Hi Paulhonda, Yes I have now stopped taking it whilst on treatment, it seems to be safer.

Hi, is your prostate followed up by a urologist? These herbal supplements may or may not work. Not sure about what firing an enlarged prostate means. I take tamsulosin for symptomatic treatment. Not that I really need it lately. I was 2 years on dutasteride. That's a nasty five alpha reductase blocker with ugly side effects. It is to be prescribed by urologist. It made my PSA drop in half but as soon as I stopped taking it it rocketed back up in 6 months. I hope I won't have to take it ever again. For now it seems like my PSA is kept in check by my diet. I have no symptoms. I would not know that I have an enlarged prostate if I did not know it already. Depending from how large your prostate is and how severe your symptoms are your best option is to see a urologist if not yet.

Hi LeoPa, Thanks for your reply, I was under Hospital Urologist for many years, my PSA was 7.2 in my mid fifties, because it remained fairly stable at around 7.6 PSA they decided to transfer me back to own doctor (GP) who now keeps his eye on the PSA the last reading was 9.6 at the end of 2023, I am 75 years old.

The hospital have previously done physical examination - said was very enlarged and also did a Cystocopy which was ok.

The "firing" was autocorrect which I failed to spot and should have been "for".

I was hospitalised and catheterised 2 years ago being unable to pass any water, it turned out to be a water infection, I think they said e-coli.

I have episodes where I struggle to pass water for various reasons, either the enlarged prostate or water infections, it was at one of these that hospital doctor recommended taking Tesco Cranberry tablets and I found that the Saw Pimento was another herbal tablet that worked, if I miss the Saw Pimento for a week I struggle to pass water, so in my case it appears to work.

Within days of my Haematologist recommending I stop taking it I was struggling to past water and having urgent frequency. They have tested my urine and on the simple tests that they can do they have said it contains blood so it has been sent off for full testing 2 days ago and I am back at the hospital on Monday.

I am interested to know you control it with diet? Although I am fairly restricted on what I can eat as I had bowel cancer in 2004 and around ⅔ of my bowel was removed.

Thanks for your help. Cliff
