I am advised by my dentist that I need a back molar extracted. She is questioning if she can carry out the procedure at the dentist practice or if I need to be refered to the hospital. Her reasoning is my immune system. She has only been in the profession for 3 years, so might be aiding on the side of caution with me.
My whites are around 10, Neutrophils 6, platelets 133. On Ibrutinib, which I would stop for a few days prior and afterwards. (23 years of Cll).
Given the above do I really need to go to the hospital for this extraction?
Could others share their experience?

I have had a few extractions while I was on Ibrutinib and my Onco had me stop 3 days before and I started back on the second day after because there was no bleeding. If you want you could ask for an antibiotic. There was no problems with the Ibrutinib starting to work again and I didn't notice any difference. Oh by the way they were all done by a dental surgeon, and not in a hospital.
I had two wisdom teeth extracted on the same day a few years ago at the dentist office. I was in remission, but my blood counts were still low after FCR. Everything went well. I'm not sure about the Ibrutinib though. Stopping for a few days before sounds like a good plan.
As back molar I would agree with caution and get a speedy referral to your nearest dental hospital, given the possibility of bleeding. Not sure who does the referral. I will have this issue at the back of my mind now because of osteoporosis I should have extractions done in a hospital setting. Sadly just another hurdle to jump.
I have had issues with my teeth for which I blame CLL. My own dentist will not take responsibility of an extraction and refers me on. I have now had 6 extractions mostly by hospital. Each time it has been a long drawn out costly process. I wish you luck.
