Should one stop taking Ibrutinib if receiving Sotrovimab or Paxlovid for COVID?

Just a precautionary question.

Hi RobertCLL,

-

We have had several recent discussions about the interaction of Paxlovid. The bottom line is that Paxlovid is taken for 5 days, and during those days only (no pause before or after) your doctor should modify your dose to reduce or pause taking Ibrutinib.

Does this also apply to Venetoclax? It would appear so from the drug interactions reference cited?

Yes,

It applies to all the CLL drugs that have a "grapefruit" warning ( to avoid CYP3A inhibitors). Paxlovid will cause those drugs to act much stronger and stay in your blood much longer, possibly making side effects worse.

Many thanks for such a quick reply. I am hoping not to require any of these measures as I am living like a hermit whilst going through first line O& V treatment but better to know just in case so it looks as if it would be sotrovimab for me or a pause in Venetoclax if doctor advises. Think I will maintain my undercover status till treatment is over.

Hi Sumoldbloke,

-

There seems to be some indications that the latest sub variant can overcome sotrovimab, so Dr. Furman is recommending Paxlovid for COVID positive / exposed patients and Evusheld as preventive / prophylactic. -

Many thanks for the replies. Just spoken to my GP, and having my recorded amended to state that Paxlovid is contraindicated for me. Therefore, Sotrovimab should be administered.

As I said just a precautionary activity, just in case.
