I have been taking Imbruvica 420 for about 4 years. I have been having muscle and bone pain, my doctor lowered my dose to 280. I again have pain. Can anyone tell if these go away in time even if I stay on Imbruvica.

Try Claritin without D....helps with pain for many. Do you see a CLL specialist?
Ask your specialist about switching to Acalabrutinib/Calquence. It does have a lower and somewhat different side effect profile. There's of course no guarantee of an improvement and unlike Ibrutinib, the only way to reduce your dose is to switch from taking 100mg twice daily to once daily. This has the disadvantage of not providing the same amount of cover over the day, due to the shorter half life of Acalabrutinib compared to Ibrutinib, but your CLL should be well under control after 4 years.

My joint pain decreased to 1-2 episodes monthly after the first two months of Imbruvica. I continued to experience knee pain 2-3 day episodes during the 1 1/2 years of Imbruvica. 20 minute ice pack sessions were helpful.
