CLL, Bone Marrow Transplant and Hodgkin’s Lymphoma

Can’t really answer this as I haven’t had one for my CLL. Am 76 years old. Issue of bone marrow transplant has been raised several times in my CLL “career” ( 16 years ). My specialists have told me that I am too old for such a procedure. In fact one of them said it would probably kill me. And that response was about 10 years ago.

Current concern is an additional diagnosis about three months ago confirming that I now have Hodgkin’s lymphoma. I suspect Richter‘s Transformation from the CLL is at work here. Not sure. My haematologist says that he’s not sure either. Currently doing chemo for it. Chemo seems to be working quite effectively re: lymphoma and CLL seems to be in remission!? Doing it at Sunnybrook, Toronto. Problem is that the chemo sessions will now go until the end of October. Chemo side effects. I hope that I can make it until that time. I will. Anyone have a similar profile?

Very similar case to that of my cousin. He has follicular lymphoma, another of the often chronic, incurable blood cancers, and transformed to DLBCL (diffuse large b cell lymphoma) a little over a year ago. He is your age. He was treated at the MAYO back east with RCHOP, which has taken care of his DLBCL. He’s back on his bike working up to the 150 mile fundraising ride that he does every year.

Do you have the option of having other specialists consult on your case, either doctor to doctor or directly with you to pin down a more definitive diagnosis? That probably has already happened. I know that Sunnybrook is a well respected center. When my cousin’s son was going through a transplant (blood cancers run in my family) I joined a group that focused on both Hodgkin’s and non- Hodgkin’s. The treatment you listed in your profile is the one most Hodgkin’s patients get.

I have SLL/CLL that has transformed to diffuse B cell lymphoma --Richters Transformation. When I first got the lymphoma in 2017 they treated it with R-Chop (80% success), then RGP followed by a Auto stem cell transplant in 2018. That gave me a remission but it all came back at the beginning of 2019. It has now mutated again from CLL to diffuse B cell in the last couple of months. At first a Donor bone marrow transplant was on the table but now they are looking at CAR-T therapy to deal with the lymphoma. I am in my 50s but age was never mentioned when they said CAR-T was my best chance of getting the lymphoma under control. I am pleased to hear that R-chop is working for you. Mine did initially as well but then stopped as my disease mutated. So it might be worthwhile asking your doctor about CAR T therapy. I have read where people go to get their T-cells harvested when they are healthy in case they are needed for CAR-T therapy at a later date. However I don't know where this occurs.

I have a friend who will be doing a stem cell transplant at the Mayo Clinic and is having a hard time getting care givers for after the transplant she has to stay at at transplant house for 3 months close to the hospital. Any suggestions??? please😊
