As a main carer what do you put in place for a loved one if you go into hospital?

Going into hospital is a worrying time for any of us. When you are a carer this can be increased ten fold, the planning beforehand is like a military operation. I have found Carers will often put themselves last, the thought of making arrangements for their loved one can be to stressful but at times it is unavoidable.

Planning these things in advance can be hard to think about but is essential for those caring for vulnerable people. Plans for respite either at home or in care homes are a good start. Building relationships with relevant care providers and talking to other family members is important. What do you do?
