hi so I have had fluctuating high ALT since 2014, with the highest being 400+ in January this year. I’ve had 2 liver biopsies both of which came back normal I’ve had 2 fibroscans which showed 3.5 in 2020 and 4.5 last month (2025) so still within limits . I’ve asked to change hospitals as my consultant still hasn’t found a cause. I’m itchy and tired all the time . Last month after a fall I had a ct done which showed an enlarged thymus gland so they were querying thymus hyperplasia, however my liver specialist completely dismissed this and wouldn’t even look into it as another hospital did my scan.

any suggestions ??

I take it they have run auto immune antibody tests and ruled out an Autoimmune case to you issues. AIH certainly 'flares' with increased ALT being commonly elevated.

Yes I believe so back in 2014/2015. but it’s not just the ALT that’s raised . And yet still I don’t have a reasoning for it or actual investigations regarding auto immune.. I know they’ve tested for thyroid but that comes back fine . I’m just at a loss .

I only drink alcohol on the very rare occasion (like a beer or 2 on Xmas and new year)

The other tests fluctuate high as well , in january 2025

Serum alkaline phosphatase level was 301

Serum alanine aminotransferase level was 111

It was the Serum gamma-glutamyl transferase level that was 410
