Liver Biopsy report, can anyone interpret?

Hello all, got my cirrhosis dx in march from a biopsy, however I have the full report, is there anything else of note from it other than I have cirrhosis from fat in the liver? The biopsy followed a high fibroscan read of 30.7kPa.

Clinical Information

Macroscopic Description

Liver - brown core, measuring 24mm. Bisected at 20mm. 2[1]nr

Microscopic Description

Adequate liver biopsy demonstrating some distortion of the vascular relations with closely approximated portal tracts and central venules. There is portal and parenchymal fibrosis with bridging septae, rounding up of cell plates and occasional small complete nodule formation.

The portal tracts contain a minimal and focal lympho histiocytic inflammatory infiltrate without interface activity. Central ducts appear normal and there is reactive ductular proliferation only.

There is mild macrovesicular steatosis associated with ballooning degeneration and Mallory body formation. Occasional small histiocytic aggregates cell present representing lipoma granulomas but there is no necroinflammation. There is no cholestasis.

There is no deposition of copper, copper associated protein, alpha-1 antitrypsin material or siderosis.

SAF score for Non-Alcoholic Liver Disease

Steatosis (0-4)

1 - 5%-33%

Microvesicular present

No

Activity 0-4, sum of following

Ballooning 0-2

2 - rounded cells, twice as large, clear cytoplasm

Kleiner (If definitely ballooning)

1 = few

Inflammation at x200 magnification 0-2

0 - none

Portal inflammation

0 = none/minimal in some

Fibrosis 0-4

4 - cirrhosis

Ishaked if > 2 fibrosis

6 - cirrhosis

FINAL DIAGNOSIS

Definite Steatohepatitis

Conclusion

Native liver biopsy: Steatosis with ongoing steatohepatitis and cirrhosis consistent with NAFLD and/or alcohol as the aetiology. There are no additional findings.

Hello, none of us on here are experts and may interpret it wrongly. It would be best to chat to your consultant, gp or who whoever requested the biopsy for reassurance.

I don't think there is anything more important there than the final diagnosis bit which is what you already know :-

Hopefully you are working on having a healthier diet and more exercise which will help reduce the fatty liver aspect of your condition and hopefully help to prevent further worsening.

You've had great advice here above but it's now important to remove any additional stressors on your liver. So if you haven't already cut right down, or preferably cut out alcohol entirely, consider your diet (lots of fruit and veg, good sources of protein, minimal high fat, highly processed food), gentle exercise. Nobody is asking you to run a marathon every day and eat nothing but chicken and broccoli, but a walk or a swim as often as you can manage.

Your liver is the most forgiving of all your organs and can, to a certain degree, improve and repair itself, but only if you treat it nicely 😊
