Hello, I had a fibroscan and the result were 6,4 KPa F0, and CAP 204, should I be worry ? I will see my doctor (due to covid) until the the end of this month.Can I improve my number ? I stop drinking about 23 days before my test. Please any advise is very welcome, Thanks

I had one last week and scored 6.5. Consultant said he isnt worried unless he sees a score over 8. He said at 6.5 its considered "normal" but I may want to consider laying off the booze for a while to let me liver "recover" as there has been some mild scarring given the score is over the 5.3 average. He said there was no point redoing the test for the next 3 years unless I start drinking heavily.

Personally now, I am not drinking at all, I am not missing it, and I intend to not drink at all this year. This has been a wake up call and im taking notice of it!

Thank you for the reply, Did you doctor mention if the damaged done could be reverse if you go alcohol free from now on ? in my results mention F0 ( no fibrosis) but as you said still high

There isn’t permanent damage I was told at that score and yes reversible. Your CAP score also doesn’t indicate fatty liver so just stay off the booze for a while.

Thank you! feeling much better now, my CAP number was 204 which is on the normal range, but have been eating better and no more alcohol for me now. wishing you all the best

Your CAP number is normal, so that probably doesn't need much improvement (if any at all), and your KPa score sounds borderline, so it sounds totally reversible. Of course, that should not be construed as official medical advice.

The key to reversing the damage is confirming what caused the damage to begin with. You mentioned stopping alcohol 23 days prior to the test, but is alcohol the main culprit, or the sole culprit? I'd also talk to your doctor about any medications you are on or plan to be on, and be careful with supplements or Tylenol, etc.

Assuming alcohol is the sole culprit, you'll probably have to keep that to a minimum for a couple months. I'd talk to your doctor about how long you'll have to avoid alcohol, and how much is safe to drink.

How much were you drinking before the 23 days, and for how long? And what were your blood results like?

Thank you very much for your reply, alcohol is my problem I don't take any medication and I don't do drugs, the last 10 years my problem has got worse, august last year I started to count my units an I was between 70-50 a week, at the beginning of 2020 my liver enzymes were high GGT218, ASAT 69 ALT 79, at the end of Sep I started to work on my anxiety, I have been reducing ever since, some weeks ha been better than others but I went down to 30-15 units a week an since the 8 of feb I have been alcohol free, the last week in January I went to the hospital because stomach problems and they took blood test and everything was normal, after that I got referred to the gastro, he wanted to see if any permanent damage has been done, it is scary and I wish I have work on my anxiety before instead of using alcohol to cope with it

My result was also 6.4. I had one hepatologist tell me yes I had some minor scarring and another one tell me I have no

scarring. My cap was 300 and the report said S1. I questioned my Dr. about this as my last fibroscan of 293 said S3. So how could my CAP go up, but my steatosis score go down? He told me CAP was basically useless for quantifying the exact amount of fat and that there are several guidelines and studies regarding CAP, so depending which guidline your Dr. chooses to use it will determine your CAP interpretation. He said only an MRE is accurate for quantifying the percentage of fat. He did say Fibroscan CAP is ok for determining if you have fatty liver or not and that with my scores of around 300 I did have fatty liver. I was a drinker, but pretty much stopped 3 years ago. This Dr. said I could have a drink as long as I keep it in moderation and it was very important to keep my weight down and continue with exercise. I lost 40lbs and I am now an avid runner and Peloton biker. I still don’t know who to believe though. The first hepatologist who said minor scarring or this one who said I have no scarring.

Did the doctor who said your liver has minor scarring mention if you could reverse the damage ? or gave you some diet, advise how to help your liver to recover? by the way well done, 40 lbs is a great achievement.

She said getting rid of that minor scarring would depend on how long the scarring was there. If it was new scarring I could reverse it. I just saw my new hepatologist last week and he is the one who said I had no scarring. When I asked why my last Dr. said I had minor scarring he really wouldn’t say. He just shook his head no and said I had no scarring. I never got any advice as to diet. I was always an athlete and knew what I needed to do. I run half marathons now and my bmi is 25, down from just over 30. I am just at the healthy range now. This new Dr. said I am at very little risk for progression if I continue on my path. He said I did not even need yearly fibroscan anymore unless I wanted one, which I do. I am waiting on my ultrasound result. My last one was normal and the one before that when I was heavier indicated the fatty liver. The Dr. explained a normal ultrasound does not mean I don’t have fatty liver and that it could not indicate it if you have under 30% fat. He felt with my CAP of 300 I still had fatty liver and I probably always will, due to genetics. He was confident it was not NASH, my enzymes have always been normal for what thats worth.
