Pbc elevated Alp

Hi everybody, i am new here and at the moment am panicking). After 4 weeks of taking urso my blood work improved. Alt from 100 to 40, ast from 70 to 30, ggt from 450 to 150, alp from 300 to 200.

Today is 6 weeks of taking urso and i had blood test done. My alp increased from 200 to 250, ast from 40 to 58.

Do i have reason for to worry at that moment?

Thanks a lot for your reply. I didn’t ask for interpritation of my lab results and sure ,on monday will get a contact with my liver specialist in Holland.

I wanted to ask if anybody else here had ALP ups& downs in the first monthes on urso.

Hi, are you signed up to the PBC foundation, or their health unlocked forum?? They have excellent up to date evidence based information.

My understanding is that you need to have a year of urso to truly say if you are or aren't a responder. Alp is not liver specific, and can be raised due to bone changes in the body, it is normal to have ups and downs. The EASL guidelines on diagnosis and treatment of PBC aim for alp to be under 200 to say you have responded to urso, however this will be demonstrated by a general trend over months not weeks. If the urso fails to reduce alp sufficiently then the next line treatment with OCA may be considered.

Personally I wouldn't worry to much about little alterations, in the UK doctors are likely to leave it 3 to 6 months before checking bloods, to give the medication time to work. Also ensure you are taking the correct dose for your weight, that is important as many people don't take enough.

Wass71 has given you some good advice, so please don’t panic. I’ve had many ups and downs with my bloods over the 10 years since I was diagnosed. There are such a lot of things that can influence your results, your doctors will be monitoring closely that’s the important thing to remember for now.

Do Join the PBC foundation it’s based in the UK, but has members from all over the world and the advisors are happy to call or Skype you if you feel you need to talk to someone.

You'll probably find that your test results will vary lots, even with URSO. You are though always being monitored so if your consultant team think this is an issue then they will review your treatment accordingly. You'll probably find that they wont do too much very quickly. But do raise it with them next time you go and see what they say.
