Hiatius hernia update (A&E) visit

Good evening all.

I rang the Drs this morning all appointments were taken but the Dr did ring me back. The Dr has given me more lansoprazole 30mg and advised I take one twice a day. She also prescribe peptic liquid, Aniseed flavour to take after food and before bed. If I see no improvement by Friday I am to ring back and she will refer me to a gastroenterologist.

I’m happy with this and hoping it all goes away before the end of the week.

I have a small hiatus hernia (as well as reflux disease) and so I suggest you avoid bending and restricting your abdominal area wherever practical without getting paranoid about it, which I found by accident to act as a trigger for an upset a few years ago. Medication will probably keep things under control for most of the time (remembering that PPIs and antacids only treat the symptoms not the cause), but if you do have a relapse you may have to wait for a week or two for things to settle down again, and during this period you will help yourself by ensuring you chew your food properly, you restrict your meat intake, you avoid rich and spicy food and alcohol, and also gas producing foods and drink like onions and fizzy drinks. Finally do not forego the opportunity to see a gastroenterologist if is it offered, since the outcomes from reflux disease and/or a hiatus hernia can be serious in some cases and should not be dismissed lightly.

hi, I have same issue result in Barrett’s disease, I must stress the fact that diet is so important I found drinking carbonated drinks made thinks a lot worse as it forced acid back into throat l. I no longer drink these or lager etc every now and then I will treat my sled to a guiness as this is not as gassy. I also found eating several smaller meals with nothing fried during the day a lot better I tend now to eat more whole grains lentils etc and more if a Mediterranean style diet this has had a positive effect I was on 50 mg of ezopralzo twice a day this is now down to 20 twice a day which is a positive step for me and with the added weight loss it has been a bonus.

I have the odd treat like fish and chips but I tend to have the fish grilled rather than in a batter and small portions of chips I think it tastes far better this way I can’t stress how much change in diet has had a positive effect for me

yeah but ppis stop you from absorbing nutrients calcium etc, I have a hiatus hernia but have to take them.

Irrespective of whether PPIs do inhibit the body's ability to absorb nutrients or not they are essential for treating reflux and hiatus hernia conditions. However I have been taking PPIs for over 15 years and do not seem to suffer any ill effects.

I've been taking omeprazole for the last 6 years after my heart attack to protect my stomach from all the medication, Ive had no problems and now it seems I need them for other reasons as well. I am Happy to continue them as this morning the pains have almost gone, just a slight niggle now. Thank goodness,

Hi I have a hiatus hernia and acid reflux now for years and was on omeprazole with no ill effect for years (all blood tests normal and even a bone density test was normal). After my stent i was switched to Pantoprazole and then Lansoprazole none of which are working well. I wake up most mornings with tummy cramps and pain and have seen a gastroenterologist and am due another colonoscopy soon. My cardiologist said he had no issues with me continuing on Omeprazole as their interaction with Clopidogrel was not proven in that study but I always err on the safe side. I also take that aniseed medicine but its not as effective overnight. Might see my GP about taking Lansoprasole twice daily for a while and see if it helps.

All PPIs appear to have some interaction with Clopidogrel, to varying degrees, see here.

I was taking Esomeprazole (Nexium) before my NSTEMI, but when I was prescribed Clopidogrel as a follow up I was changed to Lansoprazole and although my Clopidogrel was stopped after 12 months (although I continue on aspirin) I remain on Lansoprazole which fortunately causes me no problems.

To my mind there is a balance between taking clopidogrel, which to some extent safeguards your heart health due to its anti-platelet properties and acts as a form of 'health insurance', vs. a PPI which helps you live a normal life and not suffer from digestive issues. If I was forced to choose it would be looking for alternatives to clopidogrel with my GPs input, or worse case dropping it and take the risk, rather than suffer a day-on-day issue with digestive discomfort which might result from taking the 'wrong' PPI.

Yes so it seems although the advice is to avoid Omeprazole and use other PPIs Lansoprazole or Pantoprazole. The truth is they really do not know the extent or even the significance of mixing PPIs and Clopidogrel. Apart from the horrendous digestive discomfort, without the PPI there is a much higher chance of raising an ulcer or having a tummy bleed. Then you would need to stop any antiplatelet therapy at least until it was healed. Not ideal I would think. I will stick with both I think.
