I haven't rented one in quite some time, but I have a PM/ICD, and I don't know of any restrictions. I am just curious about a driver's license and how that would work. Never really thought about it because I've never driven in a different country, always taken other forms of transportation.
Hi firstly make sure you have adequate travel insurance, if you have a current UK driving licence then there is no problem as it does not show medical issues and they don't ask questions

Because I am Type I diabetic I have to renew my licence every three years. They also removed the C1 and D entitlements fro Type I s about 20 years ago unless you pay for a yearly medical! ☹️ When I was in Australia some years ago they were only interested in seeing I had had a full licence for over a year with no serious offences. This was a worldwide hire company.
I travel to the USA frequently and rent a vehicle every time and have never been asked about health issues. Book your US rental in the UK, or on a UK (.co.uk website)as the car rental agreement will generally include cover for Third Party, Collision Damage Waiver and Theft. An excess is not normally charged in the US as it provides additional liability insurance. Stick with the big name companies like Hertz or Alamo. Apply for an international drivers license at the Post Office. Some car rental companies are now requiring it with foreign licences. I was refused a car rental pick up on my USA trip in June by Enterprise as I didnt have one but was able to rent via Alamo next door.
I would recommend hiring a car with SatNav if you are not used to driving in the USA or on the right hand side. It will make life a lot easier.
