Tooth extraction while on Eliquis
I had a wisdom tooth removed about 4 years ago and it was difficult to get advice. I take Eliquis, we call it Apixaban in the UK. My regular dentist said ask my Doctor, my Doctor said ask my Dentist. At the time, neither the Arrhythmia Nurse or my old INR nurse were much help either. In the end, I decided to miss the the morning dose and ironically, the specialist Dentist who carried out the extraction said it wasn’t necessary to miss the dose. The whole thing took minutes and although it had been a while since I’d had an extraction, I didn’t bleed excessively.
Below is a link to NICE advice on Apixaban covering all aspects of the medication which you might find helpful. NICE's role is to improve outcomes for people using the NHS and other public health and social care services here in the UK……

Good luck with the extractions……
I had exactly the same issues. Didn’t have an extraction but wanted the advice in case. Got sent round and round in circles. They all want to pass the responsibility on to the next one……..
I take Apixaban and have had two large teeth removed on different occasions.
The first extraction was planned for 8.15am, so I took my Apixaban at 9.30. No problems, slight seeping as usual after extraction. The second was unexpected - tooth broke on a nut so it was removed immediately four hours after my Apixaban dose. Again, no problems and only the usual slight seeping.
My dentist said it isn't a problem. They like to know you are on anti-coagulants but they just check a little more carefully. It doesn't really make any difference to them...
Be prepared for lots of bleeding. I was given a lot of gauze but I bled for hours, halfway into the night. Maybe do some reading and whether to apply ice or how much pressure. I reported it to my dentist and he wasn't concerned. No worse for the wear afterwards.
I had a tooth out recently and just missed the morning dose / no harm done. This is why I pushed for APIXABAN over other once a day anticoags - it gives you flexibility and leaves your system pretty rapidly. It worked for me.
This is all very interesting to me. I have to have two teeth removed, separately and my dentist wants me stop Apixaban for 2 days prior. They will both be surgical extractions. Personally, I think this is overkill, but my GP has said it is ok.
I must admit I am nervous about stopping it for that long.
My dentist asked what advice my doctor had given and because I'd received none and was there awaiting the extraction, he took responsibility, and went ahead. I was given extra time following the extraction to sit and bite down on a swab and then re-seen to check that bleeding wasn't excessive. I was advised not to do the usual things that are taboo following extraction (hot drinks, chewing that side) etc., and give the usual patient advisory sheet about continuing bleeding, but on the bottom, a handwritten sentence advising me to be extra sure to go to A&E (Emergency room) if bleeding didn't stop, or recommenced excessively. (It didn't, though I was nervous for a little while). But as with other bleed incidents while on Eliquis (Apixaban), once the bleeding tailed off, that was it!
Stop taking it 2 days before. Your dentist should tell you this, if not your EP should.
Think you should read the NICE guidance by clicking the link on my original reply. They are the “experts” here in the UK and they have a different view…….
This is what my oral surgeons and dentists have always suggested and to run that by my EP, and they concurred.I'm in the USA and perhaps there is different thinking regarding that here.
I too had wisdom tooth removed. Dentist said no need to stop Apixaban. To be on the safe side I adjusted my dosage times so that my next dose was due after the extraction. No bleeding.
I had two separate dental implants necessitating tooth extraction, and in both instances had to go off Xarelto (similar to Eliquis) for three days -- two before, and the third the day of the extraction.
Hi DuckWing. It is a variable. I have had two extractions but each tooth at a different time. I was a 'bleeder' on each occasion! I had padding, sitches annd two trips to A & E the first time. I found the NICE guidelines vague. Fortunately I had a good heart Consultant who when he knew of my difficulty told me what to do the next time, and put it in writing for the Dentist. I stopped three doses before the extraction and one after. I was still bleeding and so stopped another dose. I called the Consultant who told me that I had done the right thing and just to be certain to stop one more dose. Still here.TerryW.
I recently had a wisdom tooth removed and take apixaban my dentist was happy to remove without any interruption to my apixaban but he did put stitches in afterwards as a precaution and insist that I went back after 2 days just to check he removed the stitches at day 7 with no issue.
Have had a couple of teeth removed while on apixiban and no real problems.
I had a tooth pulled and screw for implant put in a few months ago. Dentist suggested two days and said ask dr. Dr said 1 day was enough. I take xarelto and had no issues at all. Good luck.
