My MD says I would need to go off rivaroxaban briefly if I have an epidural injection. Does that mean I would transition temporarily to warfarin like I did for ablation? Or be completely off anticoagulant? I read that stroke risk is higher just after stopping anticoagulant.

From what you say and some experience for a lumbar puncture test then probably off rivaroxoban but not going onto warfarin. You need to ask specific questions and also if they are going to do bridging before / afterwards (not sure it's possible / done re the NOACs. With warfarin obviously longer time to leave and then after to get back in system.
You would only stop the rivaroxaban for a day. I had L4/L5 injections and was just told not to take the tablet the night before. I think rivaroxaban is only in your system for about 24 hours. Then restarted day after jabs when no bleeding had occurred. Dont worry. Talk to your cardiologist too.

It's all about balancing the benefits and risks. Yes there are increased risks from stopping the rivoroxaban, but epidural injections would not normally be undertaken if there is any increased bleeding risk (as it would increase the chance of an epidural haematoma) so you will need to stop the rivoroxaban if you want the epidural. All good advice there. Less of a problem if one is in NSR I would think. I recently had to stop Rivaroxaban for a biopsy procedure and was (wrongly) told to stop it 10 days before. Luckily, all was OK and I restarted after 5 days. Even more recently, I was experiencing a bleed through a urethral catheter. I missed that evening's tablet and overnight all was OK again and the next tablet was taken on time.
It's correct that they have a 24 hour active life and begin working as soon as you take them.
