hi all . following my last post about inr . well i was on 6 mil . and had blood test .it was1.8 . so they said stay on 6 mil .

had another test mon and it was 1.4 .so now they said take 7 mil. and have another test mon ? the way they work it seem all wrong this time round . was on warfarin for 6 months around my first ablation . and was naver more than a few points out of range . but this time i cant get in range. strange

I'm having trouble with INR too - it's not high enough, yet the dose goes up only a fraction week by week. I'd like to get it a bit higher so that a wobble won't take me out of therapeutic range.

I don't know whether this info.( from a web-site) pasted below, would help- I showed my GP, who heads the anti-coagulation clinic, and she agreed it is correct- however, some other research says don't change dosage much in stable patients. I have found I need to change to get back in range but i monitor very carefully after increases. I seem to get an initial small increase straight away but the effects of a few days increase can still be seen 7-8 days later so I check quite carefully to ensure it doesn't increase too much..

I seem more susceptible to slight increase in vegetables than some others who have posted their experiences with warfarin on our forum.

NR²Action

>10.0Stop warfarin. Contact patient for examination.

7.0-10.0Stop warfarin for 2 days; decrease weekly dosage by 25% or by 1 mg/d for next week (7 mg total); repeat PT³ in 1 week.

4.5-7.0Decrease weekly dosage by 15% or by 1 mg/d for 5 days of next week (5 mg total); repeat PT in 1 week.

3.0-4.5Decrease weekly dosage by 10% or by 1 mg/d for 3 days of next week (3 mg total); repeat PT in 1 week.

2.0-3.0No change.

1.5-2.0Increase weekly dosage by 10% or by 1 mg/d for 3 days of next week (3 mg total); repeat PT in 1 week.

<1.50Increase weekly dose by 15% or by 1 mg/d for 5 days of next week (5 mg total); repeat PT in 1 week.

Very useful. This explains why my dose goes up by only 3mg (well under 10%) per week. Thanks!

Hi Moggdog

What Rosy posted was really interesting and useful, but I am wondering out loud, (as I am like you and struggle to stay in range currently) if they take into account previous stability, both you and I were previously stable, and perhaps they over-ride the advice thinking this is a short term aberration?

But for me, they appear afraid to drive us up to 3 when actually most of us would be safer around 3.0
