Reresearch indicates that the use of estridol patches while on adt may mitigate some of the side effects of adt. My question is on the dosage of estradiol patches. Does anyone have information or experience with the proper dosage of estradiol patches to reduce adt side effects ?

The important thing IMO is that estradiol [E2] should not be below 12 pg/mL, since bone loss can be rapid.
One should perhaps aim for 20 pg/mL.
The lowest dose E2 patch will do the job & it may not be necessary to use it daily.
I believe that the lowest Vivelle-Dot patch is 0.025 mg.
It will not increase the risk of a blood clot.

I tried ADT solely from E2 patches per the revised PATCH trial protocol. This used 4 patches and changing one out every day. They are semi weekly (4 day) patches and the strength is 0.10 mg/24 hours.
That brought my T down over the course of a month but not to below 20 my/dL. So I punted and took a shot of degarelix to promptly get me there. To my delight and surprise I had none of the severe SEs I had on previous ADT. So I am a believer in Estradiol with ADT
Now to answer your question, I reduced my dosage and found I could use just one or two patches at the 0.10 strength and change them out every two days. Not 4 days as most of the estradiol is released and absorbed in the first 2 days. Your mileage may vary.
Personally I do not worry about my E2 level being pretty high if my PSA is low and not rising. If breast swelling and tenderness begins developing go on tamoxifen 10 mg daily to prevent gynecomastia.
Ive been using patches for almost 6 years now and they work great, I used to have really bad hot flashes, now I get them now and then but no where near what they were. I use 0.1 mg twice a week, originally prescribed by Dr. Myers but my other docs haven’t had any problem prescribing them now that he’s retired.

Shouldn’t be an issue getting a script, if you search on line there’s plenty of data supporting patches to help with side effects. And they don’t cause coronary issues because they are absorbed through the skin unlike pills that are absorbed through the liver, so don’t let them give you that line, they should know better.

Dr. is willing to prescribe the patch but somewhat uncertain as to dosage. Said he will research . On your usage of the 0.1mg twice weekly what is frequency of patch change out? As example, one on for 24hrs Tue and the second on 24hrs later in the week.
Are you on Lupron alone or with Zytiga ??
