I am a silent follower. I have been reading you all for at least 10 years. I am in Paraguay, South America where there is no access to many medicines. My situation has changed now. gleason 4+4. Radical Prostatectomy 09/2011 + Coadjuvant Radiotherapy 02/2012 + 1 year of Eligard (leuprolide) 22.5 every 3 months in 2012. PAS nadir 0.01 and discontinued. Relapse in 2018, again 1 year of Eligard, down to 0.01 and discontinued again. then in May/2024 relapse in 2024, used Eligard + Enzalutamide. PAS down to 0.100 nadir. Now April/2025 up to 0.180 (increased approximately 0.02 every month. Testosterona 27.4. I did not do chemo yet. did not take abiraterone. No Metastasis detected yet. what's next for me from here. I would like to know your opinions that were always very helpful. greetings and thank you.

forget to mention. my RP was at 51 years old. I am now 65 years old. and my Pas was never more of 4.

There is no need to do anything different until your PSA reaches 1.0 and your PSA doubling time is less than 9 months.

Thanks you tall, I always read you. And when these values arrives, if that happens, what is the tratement that you would suggest? off course i am under oncology control but i would like your opinion.

Ask me if it does. The best treatment now may not be the best treatment then.

You may think you've been silent for the past 10 years but I could hear you chomping on all those berries.....and not offering any to us...Caramba.....

well not sure I can be of much help. I had stage 4 with Mets when diagnosed. I went directly to my oncologist for treatment , my treatment went to chemo (taxotere ) 6 rounds then to xtandi since then. Everything I have been told is that if the cancer has not left the prostate then the prostate should be removed. But if the cancer has left the prostate then don’t bother removing it
