I don’t know whether it is fair to compare PSA results on different meds or not, but my husband was diagnosed May 2024 with a PSA of 315 and immediately started on Lupron joined by Abiraterone and prednisone starting July 2024 after 5 radiation treatments to T-11 and 12. Also having monthly infusions of Zometa for the bone mets. His PSA as of December was 0.06.

Medical care here in public healthcare is great.You get the best treatments almost free. For example Nubeqa costs are only 4,5€/month. Doctor appointment in hospital 66€ or 38€ in health-care center.Finland🇫🇮 is beautiful and peoples friendly and honest.Welcome
My PSA is below 0.1 but started nearly 200 times lower than yours despite "extensive metastases". You have a much better than 99% reduction, which is one of the criterion used to gauge excellent response. (In fact, much better than 99.9% response!) I personally wouldn't worry too much about a couple residual tenths.

PSA at time of Dx was 3.5. Gleason 9 with 3 lymph mets in pelvic region. 44 rounds of IMRT - 2 years Lupron and Abi. Last injection was April '24. PSA has remained undetectable.

PSA 150 just over a year ago. On Apalutimide, Megestrol and Lupron. 6 weeks radiation (IMRT) and have been holding at 0.02 PSA since last spring. One more year to go of ADT!
Still holding onto my prostate. Diagnosed 2 years ago with PSA12 and 4 bone mets. Been on Lucrin/Abi since then and my latest monthly PSA is 0.087.

I had cancer remaining in my radiated prostate, and it was growing larger in there, until I began hormone therapy. , The prostate and the cancer are still there, but the PSA is 0.01.

I still have my prostate, (my older brother had his taken out in 2013, and I thought it was just brutal) I was diagnosed in Oct of 2023, so 15 months of Lupron/Abiraterone/Prednisone and 28 rounds of IMRT on pelvic area. ending right about a year ago. My PSA has been at 0.04 since Jan of 2024.
Also had 3 lymph nodes in periaortic region that got 3 rounds of SBRT.
The original plan was 2 years of Lupron + Abi/Pred, then just Lupron or Elligard for an undetermined time.

My treatments being Oligometastatic was set Lupron - Abi - Pred. 2 years. Also 44 rounds of IMRT. My local MO and my MO at MD Anderson are hopeful that I have a more than 65% chance of a cure. So yes the prostate is still there but it has been crushed due to the treatments. My T is also still undetectable a of Oct, my next blood work is in 2 weeks.
6 years ago, 04/2019, PSA 2250. Bone and lung mets. No surgery, no radiation. Lupron, Abi, Prednisone. Within 3 months PSA<0.05. Stayed on the same regimen until 10/2023. Now off all meds for more than a year. PSA still <0.05. I am lucky and happy at age 81.
inoperable PC due to lower anterior resection and 5880 rads of radiation 30 years previous for rectal cancer. When on Lupron and Orgovyx PSA goes to 0.03 and T=6. Highest non-ADT PSA = 7.1
I was diagnosed in 2021 - Gleason 9, 4+5. I took part in a clinical trial involving a six month trial of Lupron, Xytiga/Prednisone and Niraparib. SBRT was administered at the midpoint of ADT treatment. PSA was 16 at beginning of treatment; fell to undetectable after SBRT. Was detectable at 0.01 at month 4. I am now at 30 months following conclusion of treatment and involved in twice annual visits with clinical trial team. PSA was .06 in December; testosterone has returned to normal levels. I am 73-1/2 years old.
Well, the "normal" range for a healthy prostate is 1 - 4., so I'm not sure the artificially induced low PSA is necessarily ideal (though I'm sure it's better than a high PSA!). My oncologist said, "If you have a prostate gland, you're going to have some PSA. Reducing it to near-zero isn't realistic." That seems quite different from what I'm reading here.
I agree with your general statement and in fact that is printed on every PSA test results along with a PSA range based on age. This assumes a prostrate and NO hormone therapy.
HOWEVER when you are taking ADT you are stopping the normal flow in the hope of either killing or slowing the disease progression so that is why the non ADT numbers do not apply.

This time last year my PSA was 386. Started Lupron/Abi+prednisone, 10 rounds of radiation to T2 & L2. PSA dropped super fast and has been undetectable since May. Actually getting a blood draw today so wish me luck!

My only treatment since diagnosis Feb 2018 has been Zytiga Prednisone Eligard Provenge infusions in 2019 and my PSA has been 0.1 ever since then. Never give up never surrender Leo

Stage 4 numerous mets psa was 465 when I discovered I had it adt firmagon lowest was 1.3 1.4 has gone as high as 6.58 but drops to around 2.5 again 7 years now scince I got the little creep
What did you do to get the PSA to drop after it rose to 6.58 or did it just go back down over time?
I take mebenzenole curcumin cbd oil all the time as well I'm not saying they have anything to do with it as I'm on them when it goes up but it just drops again sometimes quickly other times over several months my bloods are done here every 4 weeks had ct scan with contrast start of December no change even my oncologist can't seem to figure it out they had a meeting about me last week a group of them together not just me other cases as well so waiting for an appointment was supposed to be next Monday but no letter of appointment yet hope this helps and things work out for you
diagnosed 2 1/2 years ago stage 4 with multiple bone Mets and psa of 87. Triplet therapy of Lupron, abi with Prednizone and zytiga. For last 20 months psa and T undetectable. Still have prostate which shrunk dramatically.
My PSA was only 2.7 when diagnosed Gleason 8. It was undetectable (less than 0.1 at the lab I use) after a six month eligard shot and HIFU full gland ablation. I had that surgery 8 years ago. It has since ticked up to 3.2. I was put on dutasteride which knocked it down to 1.3. I am now using dutasteride intermittently. I have PSA checks every four months now. I am 74.
Upon diagnosis my PSA was at 104. After triplet therapy using Docetaxel x 6 infusions, Zoladex every 3 months, and Nubeqa daily, it was at 0.0 6 months later. So I'd say the answer to your title question is ZERO, but it all depends on how your particular PCa cells respond to androgen deprivation.
I notice many on this thread quoting PSA values with 3 decimal spots (i.e. 0.003). Ultra sensitive tests are nice to have, but with a prostate still in your body you're going to have SOME fluctuations in the numbers, especially if there are still some low levels of testosterone in your system. So such tests might lead to unnecessary anxiety.
sorry to say I still have that seed planter. PSA always reported out at less than 0.1.
Moments ago my oncologist said it’s been zero whatever that means for last two years. On Abby and Lupie only.
My wife took mine using a pair of Fiskars, and it now hangs around her neck in a beautiful 14kt gold plated Kalevala necklace.....But I'm lucky cause she left me my "Pallo" and my "Kulli".

I am 77 years young and had ADT(Lipton Depot) in 2020 (PSA 21.4μmg/ml.
Last PSA IN February 2024 was 0.03μmg/ml.
Nadir PSA=0.01μmg/ml.
Mine went from 49 to .04 after 6 months of Casodex and Lupron. That was 14 years ago. Since then I've used natural ADT along with Casodex intermittently evey 3 months. I'm still here and never developed resistance. PSA is table under 4.0ng.

Triplet therapy was just around the corner but I couldn't get insurance approval in those days.
Started with Lupron, 6 infusions chemo.
10 months after last chemo infusion progression and started Zytiga. During the year Zytiga was working my PSA got the lowest its ever been these 5 years (1.2).
