I read that when zytiga and deltacortene fail, replacing deltacortene with dexamethasone allows zytiga to continue to be effective. I have been interested in prostate cancer for many years but I had never heard of this and I must admit that it surprised me a lot. Does anyone know how this is possible? Are there people who have made this choice, what results have you obtained?

Yes - When Zytiga + prednisone failed then switching to dexamethasone.

I know there are many users in this group mentions that this combinations did work after prednisone failed with Zytiga.

How is it possible that a change of steroid can stop the disease even for a few months? Have you had any idea about this by any chance?

I have read that compared to Prednisone, Dexamethazone may suppress the adrenal glands more effectively so they make less androgen. And if cancer cells have become resistant to Prednisone, they may still be sensitive to Dexamethazone.

I just had a consult with Dr. Samuel Denmeade at John Hopkins in Baltimore Md. He told me that after being on prednisone for a period of time the prednisone can can interact with gut micro biome and produce testosterone like androgens which can fuel the cancer.

I am at this point of treatment very soon. My local MO said I need to start Doxcetaxal. I plan on switching to dexamethasone and if that fails I probably will enter a Bat trial.

How was Denmeade on first meeting? Or was this your first?

He was great! The first visit must be face to face but after that he does video visits if you are in a state he is licensed in. He asks questions about everything from where I worked to my family and everything I had done from the time I was diagnosed till present. He was writing notes as we talked. When I went to my portal to see what he wrote in the notes I was somewhat disappointed because there was nothing there even after a week but then 12 days later it appeared and It was quite long and very precise. He was definitely 5 star. I was pleased that he explained why switching to dexamethasone from prednisone works for many and that was written in my portal notes. if you go online and read the reviews you will read one from a guy that says”I drive 1000 miles to see Dr. Denmeade, that says it all”. No I wouldn’t but 60 miles doesn’t seem so far now.lol

If you decide to make an appointment via telephone make sure you have at least a half an hour. They will access your medical records while you are on the phone. It’s not a simple call and pick a day and time kind of thing. God bless.
