He started on abiraterone 3 weeks ago, 1000 units, plus prednisone. One week ago received HRT and Zometa at same clinic appointment
Feeling deliriously exhausted he took a 24 hour mini time off a day ago.
Amazingly…Feeling much better. Not sure if one day off occasionally will be long term health consequences. Or if the two doses in Clinic setting shocked his body and should be given at different intervals.
History: Robotic surgery 2.5 years ago. Lymph node radiation 35 treatments. 1.5 years ago. Pelvic radiation 5 treatments in Sept. .He has advanced metastatic cancer, 64 years

You could switch to a different drug, Enzalutamide, Apalutamide or Darolutamide. These are as effective as Abiraterone but have a different side effect profile. Darolutamide seems to cause few side effects.
I am on Zoladex every 3 months & Nubeqa 60mg day. On occasion I forget to take the Nubeqa, especially on gym days, & more energy is the result. It does not seem to effect the PSA reading either.
I started out at 120mg day,4 tablets, & reduced it to 2/ day so I probably go without any twice week. On those days I have a peaceful sleep I might add. Something about Nubeqa makes me wake up every 2 hrs,even taking melatonin 2mg before bed.
My dad (now 80) exactly the same in terms of side effects from zytiga. He stopped the day before chemo - first chemo was about seven years ago, doxyrubicin, We had to fight for both chemo and radiation for him. He went through both well. This chemo is docetaxel. He's feeling so much better off the zytiga it's extraordinary - despite being a week post chemo.
I would add that I did not enjoy Zometa at all, and immediately returned to Xgeva, which gives me no SEs at all. If his really bad response happened right after the Zometa dose, I would be suspicious of that.
