Xtandi failed awaiting new therapy

Hi My husband after becoming Castrate Resistant on Adt was given Xtandi for 8 months when psa dropped from 82 to 9.8 however psa then went back up to now 70 in the last 1-1/2 months. His testosterone is 0.02 .He had an orchiectomy in March this year so not taking Adt injections since then.

He has been told by Onc Xtandi has failed. His next treatment is Docetaxal however he is being genetically tested for the BRAC 1/2 mutation, if that comes back positive Olarapib will commence, awaiting results of Ct and bone scan. Should know everything by the end of the month.

In the meantime he was told to continue on Xtandi as it could be doing something? why if its failed? Psa is still going up. He has suffered with constant headaches with this drug and if it's doing nothing whats the point of taking it and having the toxic crap roaming in the system would it be better to stop taking it, he will be going on another therapy in 2/3 weeks.

He also developed high BP whilst on the Xtandi and prescribed meds never had BP problems before then. I am wondering if he should cut back the BP meds as his BP readings now are low he seems tired more than usual.

sorry to hear that Xtandi didn’t last very long, keeping an eye on your bg and bp can be important to give your medical team important feedback and for personal reassurance. Could you give us a more detailed history and medication list so we can give more informed thoughts on your husband as things move forward.

after Xtandi failed we went off right away because at that time we were paying $700/month for a drug that wasn’t working anyway. He went back on taxotere chemo, 9 rounds. But my husband had been tested previously for the gene and was negative so going chemo was started right away

That was 2022 when off Xtandi. Since he had 9x of taxotere, 4x of pluvicto, back on chemo but Jevtana this time. PSA went from 235 to 32. But since 9/12 he’s been battling radiation cystitis. Bleeding/clots from the bladder. Been in the ER 4 times, spent 15 nights in the hospital. We have an appt for hyperbaric oxygen treatment evaluation on the 22nd. Chemo has been postponed since 8/30

If MO wants him to continue on Xtandi, there are lower doses available, 120 and 80. My PSA is rising but continue on Xtandi (80mg) because when I stop, take a break soft tissue mets return so the MO is correct "it could be doing something" [IMHO] 🙏
